POLITICAL ROLES AFFECTING CHANGES IN PUBLIC HEALTH ADMINISTRATION IN LOEI PROVINCE

Abstract

This research aimed to examine (1) the decentralization of public health administration in Loei Province, (2) the political roles influencing changes in public health administration in Loei Province, and (3) the impacts of political roles on public health management in Loei Province. The study employed a qualitative research approach through document analysis and in-depth interviews with key stakeholders, including the executives of the Provincial Administrative Organization, the Provincial Public Health Doctor, the heads of district public health offices, the directors of subdistrict health-promoting hospitals, and public health officers in Loei Province.The findings revealed that the transfer of public health responsibilities to the Loei Provincial Administrative Organization was implemented in line with decentralization policies. However, challenges remained in terms of insufficient budget, shortage of personnel, and ineffective coordination. Political roles in the area had both positive aspects—such as promoting policies that promptly addressed public needs—and negative aspects, including interference in management through unfair budget allocation and unjust personnel transfers and appointments. These issues affected transparency and good governance in public health administration.The study developed the LEOI Model (Local Empowerment and Organizational Integration Model), which emphasizes empowering local authorities (Local Empowerment) and integrating organizations (Organizational Integration) to achieve effective public health management and mitigate negative impacts from local politics.The research proposed solutions including the establishment of a strong independent oversight mechanism, the promotion of public participation in decision-making, and the strengthening of good governance in local administrative organizations to enhance the efficiency and sustainability of public health administration in Loei Province.

การวิจัยครั้งนี้มีวัตถุประสงค์เพื่อศึกษา (1) การถ่ายโอนอำนาจการบริหารงานสาธารณสุขจังหวัดเลย (2) บทบาททางการเมืองที่ส่งผลต่อการเปลี่ยนแปลงการบริหารงานสาธารณสุขจังหวัดเลย และ (3) ผลกระทบจากบทบาททางการเมืองต่อการบริหารจัดการสาธารณสุขจังหวัดเลย โดยใช้การวิจัยเชิงคุณภาพผ่านการศึกษาเอกสารและการสัมภาษณ์เชิงลึกกับกลุ่มผู้มีส่วนเกี่ยวข้อง ได้แก่ ผู้บริหารองค์การบริหารส่วนจังหวัด นายแพทย์สาธารณสุขจังหวัด หัวหน้าสำนักงานสาธารณสุขอำเภอ ผู้อำนวยการโรงพยาบาลส่งเสริมสุขภาพตำบล และเจ้าหน้าที่สาธารณสุขในจังหวัดเลยผลการวิจัยพบว่า การถ่ายโอนภารกิจสาธารณสุขไปยังองค์การบริหารส่วนจังหวัดเลย มีการดำเนินงานที่สอดคล้องกับนโยบายกระจายอำนาจ แต่พบปัญหาด้านงบประมาณที่ไม่เพียงพอ บุคลากรที่ขาดแคลน และการประสานงานที่ยังไม่มีประสิทธิภาพ บทบาททางการเมืองในพื้นที่มีทั้งด้านบวกในการผลักดันนโยบายที่ตอบสนองความต้องการของประชาชนอย่างรวดเร็ว และด้านลบที่มีการแทรกแซงการบริหารงาน เช่น การจัดสรรงบประมาณที่ไม่เป็นธรรม และการโยกย้ายแต่งตั้งบุคลากรที่ขาดความเป็นธรรม ส่งผลให้เกิดปัญหาด้านความโปร่งใสและธรรมาภิบาลในการบริหารจัดการสาธารณสุข และได้โมเดล LEOI Model (Local Empowerment and Organizational Integration Model) สื่อความหมายถึงการให้อำนาจท้องถิ่น (Local Empowerment) และการบูรณาการองค์กร (Organizational Integration) เพื่อการจัดการงานสาธารณสุขที่มีประสิทธิภาพ และลดผลกระทบเชิงลบจากการเมืองท้องถิ่น งานวิจัยเสนอแนวทางแก้ไขโดยให้มีการจัดตั้งกลไกการตรวจสอบอิสระที่เข้มแข็ง ส่งเสริมการมีส่วนร่วมของประชาชนในการตัดสินใจ และการเสริมสร้างธรรมาภิบาลในองค์กรปกครองส่วนท้องถิ่น เพื่อเพิ่มประสิทธิภาพและความยั่งยืนของการบริหารงานสาธารณสุขในจังหวัดเลย
